Your AmeriHealth Caritas Delaware ID Card

Now that you're a member, you will get an AmeriHealth Caritas Delaware ID card. If you didn't get one or if you lost it, call AmeriHealth Caritas Delaware Member Services, 24 hours a day, seven days a week, at:

  • Diamond State Health Plan: 1-844-211-0966 (TTY 1-855-349-6281).
  • Diamond State Health Plan-Plus: 1-855-777-6617 (TTY 1-855-362-5769).

Every member gets his or her own ID card. Do not let anyone else use your AmeriHealth Caritas Delaware ID card.

Carry your ID card with you at all times.

Delaware Health and Social Services (DHSS) Medicaid ID card

You will also get a Medicaid ID card from DHSS. If you lose your Medicaid ID card, call Delaware Medicaid Customer Relations at 1-866-843-7212 or 1-302-571-4900.

Why you need your ID cards

You need to show your medical insurance cards:

  • At every doctor visit.
  • At a pharmacy.
  • If you visit a behavioral health provider.
  • If you need non-emergency transportation.

To get service, you may need to show your:

  • AmeriHealth Caritas Delaware ID card.
  • Medicaid ID card.
  • Other health insurance cards you may have.
