Special Health Care Programs
AmeriHealth Caritas Delaware has special programs to help you stay healthy.
You do not need a referral from your primary care provider (PCP) to be a part of any of these programs. If you have one of the health conditions these programs cover, you may be able to join.

Disease management programs
Care Coordinators are licensed health professionals who have knowledge about chronic medical conditions. If you are having trouble managing a medical condition, talk to a Care Coordinator.
Here are some signs that you may be having trouble managing a medical condition. Talk to a Care Coordinator if your condition has caused:
- Emergency room visits.
- Unplanned hospitalizations.
- Abnormal lab test results.
- Signs and symptoms that have worsened over time.
You can call a Care Coordinator at 1-844-623-7090 to get help. Your health care provider can also call a Care Coordinator for you.
Diabetes Prevention Program
The Diabetes Prevention Program (DPP) helps participants:
- Lose weight.
- Adopt healthy habits.
- Greatly lower their risk of developing Type 2 diabetes.
DPP works to help lower weight by 7%. It also helps members work up to 150 minutes of activity a week.

Quit using tobacco
AmeriHealth Caritas Delaware wants to help you quit smoking cigarettes and using any other tobacco. This includes cigars, pipes, and chewing tobacco.
If you are ready to quit, we have a program that can help you. Call the Rapid Response and Outreach Team at 1-844-623-7090 to learn more. Or call the National Quitline at 1-800-QUIT-NOW (1-800-784-8669).

Bright Start program for pregnant members
Bright Start is a program for our pregnant members. Bright Start helps you and your unborn baby stay healthy when you are pregnant. We can show you how to make healthy choices, like:
- Eating right.
- Taking your prenatal vitamins.
- Getting health care.
- Staying away from drugs, alcohol, and smoking.
- Visiting your dentist to keep your gums healthy.
We also work with your doctors to make sure you get the care you need. We will share how you can get other services, like:
- Food and clothes.
- Rides to your health care visits.
- The WIC (Women, Infants, and Children) program.
- Domestic abuse support and help.
- Breast feeding.
- Home care.
- Helping you understand your emotions.

Women, Infants, and Children Program (WIC)
WIC is a nutrition program for women, infants, and children. The WIC program provides healthy food at no cost, breast feeding support, nutrition education, and health care referrals. Enroll in WIC online or call 1-800-222-2189.
Childbirth classes
We offer childbirth classes at no cost to all pregnant members. There are no limits to the number of classes you can go to. Call Bright Start at 1-844-623-7090 (TTY 1-855-349-6281 for DSHP and 1-855-362-5769 for DSHP-Plus) to learn more or to find a class near you.
