AmeriHealth Caritas Delaware Value-Based Programs Offer Providers Customization and Collaboration

PerformPlus® contracts are designed to meet specific providers’ needs, help them deliver cost-effective care

AmeriHealth Caritas Delaware, one of the Medicaid managed care health plans serving Delawareans utilizing Medicaid and long-term services and supports, is proud to support the state’s health care transformation goals by offering value-based contracting programs to meet the needs of each provider. The programs, part of AmeriHealth Caritas’ enterprise-wide PerformPlus® suite of value-based models, align with AmeriHealth Caritas’ and the state’s desire to shift health care from a system based on quantity of services to one based on quality and value. They further aim to reward providers for delivering high quality care to AmeriHealth Caritas Delaware members.

AmeriHealth Caritas Delaware offers programs designed for many provider types, and each aspect of the contract is further customized to meet the needs of the individual provider and the patients they serve. In all cases, providers have the ability to track their performance on the agreed upon metrics using an interactive proprietary dashboard, allowing them to quickly identify and act upon opportunities for improvement. Providers who meet or exceed their program’s benchmarks can receive incentive payments and/or a share of the resulting cost savings.

• Shared-savings programs for hospitals, large health systems, and provider groups
• Quality enhancement program for primary care providers
• Specialized programs for cardiology, OB-GYN, oncology, dental, LTSS, and behavioral health specialists

In addition, for providers ready to take on risk with the potential for larger financial rewards, AmeriHealth Caritas Delaware can work with them to design bundled payment, population health, and other partial or full-risk programs.
